Submission Requirements
A complete health file must include the following sections signed by your pediatrician:
Physical Examination: A routine, up-to-date assessment of your child’s physical development and general health.
Immunization Record: Accurate verification of all age-appropriate vaccinations as required by state guidelines.
Allergies & Medical Conditions: Clear documentation of any known food allergies, environmental sensitivities, or medical conditions. This ensures our teaching and support staff can implement proper safety and emergency protocols immediately.
